Applications for Child Support Services and Information
Please use this form if you wish to apply for full services from the Child Support Services Division (CSSD). Review all information carefully. Potential enforcement remedies include wage withholding, liens, tax refund intercepts, and civil contempt. Mail or take your completed application to your local field office, or apply online.
Utilice este formulario si desea solicitar servicios completos del Programa de Servicios de Manutención de Menores (CSSD). Revise toda la información cuidadosamente. Los posibles recursos de ejecución incluyen retención de salario, embargos, intercepciones de reembolsos de impuestos y desacato civil. Envíe por correo o lleve su solicitud completa a su oficina local, o presente la solicitud en línea.
Limited Services Applications
Please use this form if you wish to apply for Wage Withholding Only (also known as Pass-Through Services). Review all conditions carefully. If you select these limited services, the Child Support Services Division (CSSD) will issue an Electronic Notice to Withhold Income to any known employer, that accepts electronic income withholding orders (eIWOs), of the Non-Custodial Party. CSSD will account for all payments received, apply them to the appropriate case, and distribute them to the Custodial Party. You must have an existing court order that allows income withholding. CSSD will not pursue other enforcement mechanisms such as liens, tax refund intercepts, or civil contempt. This type of case is a payment processing case only.
Unicamente Retencion de Salario
Utilice este formulario si desea solicitar la retención de salario únicamente (también conocida como servicios de transferencia). Revise todas las condiciones cuidadosamente. Si selecciona estos servicios limitados, el Programa de Servicios de Manutención de Menores (CSSD) emitirá un Aviso Electrónico de Retención de Ingresos a cualquier empleador conocido que acepte órdenes electrónicas de retención de ingresos (eIWO) de la Parte que no tiene la custodia. Debe tener una orden judicial vigente que permita la retención de ingresos. La CSSD no buscará otros mecanismos de cumplimiento como gravámenes, intercepciones de reembolsos de impuestos o desacato civil. Este tipo de caso es solo un caso de procesamiento de pagos.
