Program of All-Inclusive Care for the Elderly (PACE)

The Program of All-Inclusive Care for the Elderly (PACE) provides comprehensive long-term services and supports to Medicaid and Medicare enrollees. An interdisciplinary team of health professionals provides individuals with coordinated care. For most participants, the comprehensive service package enables them to receive care at home, in the community or at the PACE center, rather than in a nursing home. Clinical care that is approved by the healthcare team is provided primarily by PACE doctors and medical staff. It can also be provided through contracted specialists and other providers who can meet the individual’s needs.

To join PACE, individuals can have either Medicaid, Medicare or both, or pay privately. PACE centers are required to meet both state and federal safety standards.

PACE Eligibility

To be eligible for PACE, individuals must meet the following conditions:

  • Age 55 or older
  • Live in the service area of a PACE organization
  • Eligible for nursing home care
  • Able to live safely in the community

For those who are enrolled, PACE becomes the sole source of services for Medicare and Medicaid eligible enrollees. Individuals can withdraw from the program at any time.

Contact the PACE Organization to be added to the PACE Waiting List and to start your application.  Note: Applicants must complete the HSD-100 application and submit back to the PACE Organization.  The PACE Organization will submit the completed HSD-100 application with other required financial documentation to HSD’s Income Support Division.​
